New ‘Care Teams’ feature supports better care coordination and communication

This week Maternity Neighborhood released an updated workflow to enhance care coordination and communication between providers and their clients. Because we know that communication is heart of any good relationship, we want to make sure that every message and notification gets to the right person in a way that is both timely and meaningful.

 

We call our new workflow Care Teams, because we know that whether you have one provider or many in your practice the flow of communication depends on clarity of roles and purpose. Here’s how it works:

 

  • When you add a new client (or new episode for an existing client), you can also create the client Care Team. You can add anyone from your practice to the Care Team and set their notifications for that client.  

 

  • Once they are added, each member of the Care Team can modify their notifications settings at any time.

 

  • For example, you may want to add a consulting physician to a Care Team but prefer that the client not message directly to the consultant in order to keep you in the loop. You can add the consultant either from the “client settings” tile or directly in a message with your request for consult/referral. The consultant (who needs to be added as a user and accept terms to activate) can then respond to the entire Care Team, including the client, or just reply to you. Everything stays in the record and nothing gets missed!
